DOCUMENTS REQUIRED FOR OPERATIONAL OPERATING PERMITS FOR COMPANIES
- Photocopy of Chamber of Commerce Registration - Photocopy of Tax Certificate (or Inspection Slip if Tax Certificate is not available) showing the application address - Photocopy of Trade Registry Gazette - Photocopy of Signature Circular - Photocopy of Company Representative's ID - Company Representative's Residence Certificate (obtainable via e-Government) - Photocopy of Lease Agreement (if rented, otherwise photocopy of title deed) - Environmental Cleaning Tax (ÇTV) from Yenimahalle Municipality - Business Alarm System (installation provided by OSTİM OSB) - Industrial Registry Certificate (if manufacturing is involved) If you do not have a photocopy of your Industrial Registration Certificate; you can obtain a photocopy of the Industrial Registration Pre-Registration Form from the Ankara Provincial Directorate via https://bams.sanayi.gov.tr/ using your e-Government password, along with a Fire Department Report (requested by OSTIM OSB from the Fire Department on behalf of the company) and approved contracts (Occupational Health and Safety Specialist, Workplace Physician, etc.) for hazardous and very hazardous workplaces according to their activity code (NACE code) through the OHS (Occupational Health and Safety) Expert, Workplace Physician, etc.
NOTE: Additional documents may be requested by the Unit Directorate depending on the type of workplace.
THE FOLLOWING FORMS MUST BE COMPLETED AND SUBMITTED ALONG WITH THE ABOVE DOCUMENTS DURING THE APPLICATION PROCESS.
